Job Summary:
The Clinical Staff Pharmacist - Inpatient Services provides pharmaceutical services to hospital inpatients by optimizing the use of pharmaceuticals and collaborating with healthcare professionals to deliver high-quality patient care. The pharmacist proactively intervenes to optimize therapy, documents outcomes, provides drug information, ensures timely, safe, and cost-effective pharmaceutical services, and maintains compliance with medication management standards. The role is based in the Inpatient Pharmacy or satellite pharmacies across the hospital.
KEY ROLE ACCOUNTABILITIES:
- Reviews patients' medication orders for efficacy and safety; recommends changes to prescribers if needed.
- Provides medication information to healthcare professionals, including prescribers and nurses.
- Verifies medication orders for accuracy and appropriateness; checks pharmacy technician-prepared medications before dispensing.
- Participates in medication use committees and offers recommendations.
CLINICAL:
- Ensures medication reconciliation on admission and discharge.
- Reviews medication orders; recommends adjustments for efficacy and safety.
- Performs therapeutic drug monitoring and adjusts therapy accordingly.
- Consults with peers and supervisors on drug therapy choices.
- Participates in drug use evaluations and protocol management.
- Assesses patient health status to ensure medication effectiveness.
- Monitors drug interactions and offers alternatives as needed.
- Ensures continuity of care across care settings.
- Documents clinical activities and interventions accurately.
COMPETENCY:
- Provides education to pharmacy and non-pharmacy staff.
- Mentors pharmacy students and residents.
- Participates in training new staff and students.
- Communicates effectively with healthcare teams and patients.
- Proficient in compounding and aseptic techniques.
- Familiar with departmental equipment and software.
- Maintains BLS certification.
- Maintains professional competency.
MEDICATION SAFETY:
- Monitors and reports adverse drug reactions.
- Adheres to medication safety policies; suggests improvements.
- Supports quality improvement and patient safety initiatives.
OPERATIONAL:
- Contributes to departmental goals.
- Verifies medication orders and checks pharmacy technician work.
- Provides shift communication on unresolved issues.
- Delivers courteous customer service.
QUALITY IMPROVEMENT:
- Follows policies and safety standards.
REGULATORY:
- Attends required continuing education.
- Complies with accreditation, legal, and safety standards.
- Adheres to organizational policies and values.
QUALIFICATIONS, EXPERIENCE, AND SKILLS:
Education: BSc/BPharm or MSc/MPharm from an ACPE-accredited college; Pharm.D or PGDipClinPharm preferred.
Experience: 3+ years post-graduate experience in pediatric/women's hospital setting; recent Pharm.D graduates may qualify.
Licensure: Licensed in home country; Qatar license required upon onboarding.
Skills: Clinical knowledge, pharmacy systems proficiency, Microsoft Office, fluent in English.
