The Clinical Coder reviews Inpatient and Outpatient clinical documentation in the electronic medical record and assigns and sequences ICD-10-CM diagnosis codes and HCPCS procedure codes, in accordance with the Standards of Ethical Coding as set forth by the American Health Information Management Association (AHIMA) and in compliance with ICD-10 Official Coding Guidelines, American Medical Association (AMA) guidelines for HCPCS coding, and other regulatory requirements.
KEY ACCOUNTABILITIES:
General Duties
- Responsible for reviewing clinical documentation in the Electronic Medical Record (EMR) and assigning accurate ICD-10-CM diagnosis codes and/or Healthcare Common Procedure Coding System (HCPCS) Level I (Current Procedure Terminology) or Level II procedure codes.
- Review service orders of healthcare providers and update, delete, or modifies charges to ensure accurate capture of service utilization.
- Maintains knowledge of ICD-10-CM and HCPCS code updates, guidelines, and changes and communicates to providers affected by the changes.
- Analyzes clinical documentation to validate documentation supports services rendered and meets criteria for accurate and specific clinical coding.
- Provides information and responds to inquiries regarding clinical documentation and clinical coding to healthcare providers, Finance, and other administrative units.
- Ensures that clinical coding and Revenue Cycle Management (RCM) related activities in the HIM Audit plan are implemented and reported. Makes recommendations for audits that are related to clinical coding and RCM.
- Contribute to the identification of opportunities for continuous improvement of departmental systems, processes and practices considering international best practices, to improve business processes, productivity, and operational efficiency.
Clinical Coding
- Accurately identify and assign the principal and significant secondary ICD-10-CM diagnosis codes, HCPCS Level I (CPT) and/or Level II procedure codes and present on admission (POA) indicators for inpatient admissions following official coding guidelines.
- Analyze code assignment and sequence codes in compliance with ICD-10-CM Official Coding Guidelines to ensure proper Diagnosis Related Group (DRG) assignments for Inpatient discharges.
- Capture complications and comorbidities documented in the patient’s medical record as defined in ICD-10-CM official guidelines to ensure optimum severity of illness (SOI) and risk of mortality (ROM) scores for inpatient discharges.
- Accurately identify and assign the primary diagnosis and significant secondary ICD-10-CM diagnosis codes and HCPCS Level I and/or Level II codes following the official coding guidelines for outpatient services.
Clinical Documentation Improvement
- Ensures that clinical documentation in the EMR is complete and notifies Clinical Coordinator when documentation is missing or incomplete.
- Identify encounters that may require provider documentation clarification and/or specificity to accurately assign codes and queries healthcare provider when clinical documentation is unclear, missing, or conflicting.
- Collaborate with clinical teams and Clinical Coordinators as part of the clinical documentation validation and physician query workflows.
Revenue Cycle Management and Reporting
- Confirms demographic and financial related data are accurate and makes corrections when necessary.
- Review service order charges of providers and makes corrections to charges when documentation does not support service or Evaluation & Management (E&M) level of care.
- Responds to queries from Finance regarding charges posted on invoices or final bill of patients.
- Reviews new service lines to determine appropriate HCPCS (Level I or Level II) service codes for proposed services within the service line.
- Recommends service charge codes for new and existing services that may not have a HCPCS (Level I or Level II) code.
- Reviews existing RCM and coding related reports for accuracy and completeness. Makes recommendations on findings from reviews and identifies opportunities to improve based on their review.
- Identifies and recommends new reports related to RCM and clinical coding.
Context, Work environment And DECISION MAKING AUTHORITY:
- This role requires the incumbent to effectively and professionally communicate complete clinical documentation, clinical coding, and reimbursement requirements with physicians, nurses, and other clinical staff.
- The role requires the ability to think critically and use judgement to interpret and apply clinical coding guidelines while reviewing clinical documentation.
- Key decisions having significant impact on the function/department are referred to the direct supervisor or escalated to the manager for approval before implementation.
Job Requirements:
QUALIFICATIONS
- Relevant Bachelor’s degree in Coding or Healthcare
- Coding Credential – CPC, CCS, or other equivalent credential by recognized international body.
EXPERIENCE
- Minimum 03 years experience in a healthcare setting as a Clinical Coder
SKILLS
- Ability to understand diverse clinical documentation contained in the Electronic Medical Record (EMR).
- Excellent use of the English language (must) and Arabic language (preferred).
- Good interpersonal and communication skills.
- Ability to work well as part of a team.
